Gastrointestinal Procedures

Hepatobiliary

Clinical Indications Rule out cholecystitis, evaluation of jaundice, trauma, abdominal pain, bile leak, cholelithiasis and obstruction.  
Patient Preparation NPO at least 4 hours prior to procedure, but no longer than 24 hours. No pain meds at least 4 hours prior.
Time Frame 1-2 hours. May need additional images over the next 24 hours.  
Conflicting Examination or Medications Retained barium; serum bilirubin level above 20 mg/dl (causes a nondiagnostic exam due to poor hepatic function).  

Hepatobiliary with CCK GB Ejection Fraction

Clinical Indications Chronic abdominal pain, cholecystitis, evaluate function of gallbladder.  
Patient Preparation NPO at least 4 hours prior to procedure. (No morphine or Demerol 4 hours prior to procedure).
Time Frame 1 to 4 hours (average 1-1/2 hours).
Conflicting Examination or Medications Retained barium; serum bilirubin level above 20 mg/dl causes a nondiagnostic exam due to poor hepatic function; recent administration of narcotics may invalidate the study.

Liver/Spleen

Clinical Indications Hepatosplenomegaly, trauma, primary liver tumor, metastatic liver disease, jaundice, cirrhosis, hepatitis, hepatic abscess, RUQ/LUQ pain.

Liver SPECT

Clinical Indications Rule out metastatic liver disease, localization of space occupying lesion, focal nodular hyperplasia.

Hepatic Hemangioma with SPECT

Clinical Indications Hepatic hemangioma
Patient Preparation None. Localization of site with other imaging modality helpful.

Gastric Emptying Scan

Clinical Indications Evaluation of gastric function
Patient Preparation NPO 12 hours

GI Bleed

Clinical Indications Rule out GI bleed, acute volume depletion
Patient Preparation None
Time Frame Total time 3 hours.

Meckel's Diverticulum (Ectopic Gastric Mucosa)

Clinical Indications Rule out Meckel's diverticulum, abdominal pain, positive Guaiac test.  
Patient Preparation NPO after midnight.  Pre-treat with Tagamet if possible. 300 mg of Tagamet every 6 hours for 24 hours prior to the scan. Last dose to be given 45 minutes prior to the scan. Pediatric dose based on weight. Pediatric dose based on weight.

Genitourinary Procedures

Renal Imaging (MAG-3)

Clinical Indications Assess for renal function, obstructive uropathy, renal tumor, chronic renal disease, trauma, hypertension, renal failure, renal transplant evaluation.
Patient Preparation Good hydration
Time Frame 1 hour 
Conflicting Examination or Medications None

Renal with Furosemide (Lasix)

Clinical Indications To distinguish between obstructive versus nonobstructive uropathy
Patient Preparation Good hydration
Time Frame 30 minutes pre-furosemide renal imaging followed by IV injection of furosemide and additional renal imaging over 30 minutes 
Conflicting Examination or Medications Allergy to Furosemide

Renal Imaging with Captopril

Clinical Indications Assess for renal vascular hypertension 
Patient Preparation Good hydration. Please provide list of medications
Time Frame 1.5  hours total
Conflicting Examination or Medications ACE Inhibitors

Miscellaneous Procedures

Gallium

Clinical Indications Tumor localization (especially lymphoma), infection, inflammation, Pneumocystis pneumonia, abscess detection and localization.  
Patient Preparation None
Time Frame

1 hour each session. Patien may be imaged at 24, 48 and 72 hours depending on diagnosis.
