Nervous System Procedures

Functional Brain SPECT

Clinical Indications Stroke, cowdid stenosis, Alzheimer's disease, frontal lobe dementia, multiple infarct dementia, schizophrenia, drug abuse (cocaine), obsessive compulsive disorder, seizure, attention deficit disorder, traumatic brain dysfunction, sleep apnea, AIDS, encephalopathy.  
Patient Preparation None. Patient ability to hold still and follow directions.
Time Frame Total 2 hours.
Conflicting Examination or Medications None

Respiratory Procedures

Lung Ventilation and Perfusion

Clinical Indications Rule out pulmonary embolism, COPD, and asthma.  
Patient Preparation None. Chest X-ray highly recommended within 24 hours of V/Q scan.  
Time Frame Total time 1 hour. 
Conflicting Examination or Medications History of corpulmonale or cardiac shunt are relative contraindications.

Lung Quantification

Clinical Indications Pre-operative evaluation of carcinoma/pre-thoracotomy evaluation for percentage function of different lung fields  
Patient Preparation None. Recent x-ray highly recommended.  
Time Frame Total time 30 minutes. 
Conflicting Examination or Medications None

 

Skeletal Procedures

Bone (3 phase, limited or whole body)

Clinical Indications Assess for bone metastases, bone tumor, arthritic changes, Paget's disease, osteomyelitis, osteonecrosis, bone trauma (stress fracture), loosening of prosthesis, aseptic necrosis.  
Patient Preparation None. After injection, patient will need to hydrate well
Time Frame Total time 4 hours.  
Conflicting Examination or Medications None  

Bone SPECT

Clinical Indications Rule out bone metastases, back pain, arthritic changes.
Patient Preparation None. After injection, patient will need to hydrate well.  
Time Frame IV injection--1-1/2 hours over 2 appointments
-Appointment #1 - 15 minutes. Patient returns in 2-3 hours for imaging. Scan time--1 hour.
-Appointment #2 - 1 hour, 15 minutes 
Conflicting Examination or Medications None 

Therapeutic Procedures

1-131 Therapy, Hyperthyroidism

Clinical Indications Hyperthyroidism
Patient Preparation Nuclear Medicine consultation.  Recent thyroid chemistries. BHCG for all female patients of child-bearing age (12-55 no exceptions). 
Time Frame I-131 oral dose--less than 20 minutes.  
Conflicting Examination or Medications •Thyroid Med Discontinue List:
-Radiographic contrast agents – 6 weeks
-Myelogram – 1 year
- Antithyroid drugs (PTU and Tapazole/methimazole) – 1 week
- Levothyroxine (Synthroid) – 6 weeks
- Triiodothyronine (Cytomel) – 2 weeks 
Special Notes This is a special order isotope. If exam must be cancelled, please have Nuclear Medicine notified 24 hours prior to the exam.

1-131 Therapy Thyroid Ablation/Metastases

Clinical Indications Thyroid carcinoma.  
Patient Preparation Nuclear Medicine consultation.  
Time Frame Oral I-131 solution. Requires hospitalization. Visitation restricted.  
Conflicting Examination or Medications Discontinue all thyroid meds. Recent (within 1-2 days), serum pregnancy test (beta HCG) on all females in child bearing age. Inpatient procedure 1-3 days.

Strontium-89 Therapy, Palliation of Pain From Bony Metastasis

Clinical Indications For palliation of pain from metastatic disease to bone
Patient Preparation Recent CBC, X-rays, bone scan
Time Frame Strontium-89 IV dose, less than 30 minutes
