Procedure Guidelines

Our office needs specific information from clinicians when certain tests are ordered. Please have the following information ready when scheduling the tests listed below.

Autopsy Request

Information Needed
  • Patient name
  • Patient birth date
  • Physician 
  • Date expired
  • Place expired (hospital/home/etc.)
  • Clinical information
  • Funeral home
  • Signed permit by next of kin
    • power of attorney
    • spouse
    • parent
    • child

Fine Needle Aspiration

Information Needed
  • Patient name
  • Patient birth date
  • Physician 
  • Patient phone number
  • Clinical Information
Preparation None
Time Involved 1 hour. Pathologist will consult with patient after aspiration
Appointments made by clinicians' office only. Appointment must be made at least 72 hours in advance of requested time of procedure. Patient to report directly to Lower Columbia Pathologists main office or our Nuclear Medicine Clinic. You will be informed which location when appointment is made.

Bone Marrow

Information Needed
  • Patient name
  • Patient birth date
  • Physician 
  • Patient phone number
  • Clinical Information
  • Most recent CBC (copy of report and microslides)
Preparation None
Time Involved 30 minutes to 1 hour
Patient to report directly to Lower Columbia Pathologists main office or our Nuclear Medicine Clinic. You will be informed which location when appointment is made.
