Our billing department currently has 6 staff members who are available to assist you with billing questions and/or problems. We are here to serve you in any way possible.

Billing Office Hours

• 8 a.m. -- 5 p.m., Monday through Friday

Contact Information

You can help streamline the billing process by having the following available for your clinician at the time of your visit:

• Your insurance card for the office to copy

• Your employer's name and address

• Any medical coupons you may have for the month of your visit

• Your social security number

• Your personal contact information
  

Have you received a billing from Lower Columbia Pathologists without indication that your insurance has been billed? 

• This can occur if the billing information was not properly submitted at the time of the service order. You may

  • Call our office with the information

  • Print out the insurance information form, complete the form, fax or mail the information to our office, or

Do you need to arrange monthly payments on your Lower Columbia Pathologists account?

• Print out the monthly payment arrangement form

• Complete the form

• Be certain to sign the form

• Mail the form or bring it to Lower Columbia Pathologists billing office.
