| Patient Statements |
Patient statements are mailed every 28 days. Only the first
statement is itemized and adequate for billing your insurance company. Subsequent, balance forward statements are
not itemized, and therefore, are not adequate for insurance billing
purposes. |
| Contracted Insurance Coverage |
If you are covered by an insurance company that is
contracted with Lower Columbia Pathologists, you will be billed for only:
- co-payments
- deductibles
- claim denials
- when the insurance company sends the payment directly to you.
- If you
receive a bill in error, do not hesitate to
call
our billing department.

| Non-Contracted Insurance Coverage |
If you are covered by an insurance company that is not
contracted with Lower Columbia Pathologists, you will be responsible for
the payment of your account within 30 days, unless specific arrangements
are made with our billing office. You are asked to pay the account and
wait for reimbursement from your insurance company. If you cannot pay the
account in full and wish to make partial payments, please refer to our
application for monthly payment arrangements. Lower Columbia
Pathologists will bill primary and/or secondary insurance companies, as a
courtesy to you, if the necessary billing information is provided. |
| No Insurance Coverage |
You will be responsible for payment of your account within
30 days, unless specific arrangements are made with our billing office. If
you cannot pay the account in full and wish to make partial payments,
please refer to our application for monthly payment arrangements. |
